Before seeing our clinical team, a member of our reception team will ask you to provide us with information to enable a comprehensive assessment of your dental care needs. You will find our medical history questionnaire thorough and concise.

We ask for such a level of detail because we now understand how many general medical conditions influence the health of your mouth, either directly or as side effects of the medications taken to control them. Conversely, we also have a much better idea of how your dental health can impact significantly on your general health.

For these reasons, we ask you to complete the questionnaire with as many details as possible. If you are unsure of any questions, please feel free to ask one of our dental care team members.

We are aware your personal information can be of a private and sensitive nature. For this reason all staff are thoroughly trained to protect your privacy. If you would prefer to discuss any health issues privately with the dental clinical team, simply let one of our staff members know.
